Dear Parents:
Health requirements for kindergarteners are as follows:
1. Immunizations
Immunizations must be up to date, and a doctor or clinic record presented for verification. No child
can be admitted without a current shot record. (A waiver must be signed for medical or personal
If your child is in need of current immunizations, immunization clinics are held regularly. Please call
the Health Department at 265-7265 for the clinic schedule. Immunizations provided through their
program are: Polio, Measles, Mumps, Rubella, Diphtheria, Tetanus and Whopping Cough.
2. School Health Physicals (C.H.D.P.)
A physical exam should be done before entering kindergarten. If the exam was done within 6 months
of entering kindergarten it is valid. Many low income families are eligible for free exams. Call the
Nevada County Health Department to see if your family qualifies. The exam can be mailed to your
child’s school or brought to school during the first week.
3. School Health Screenings
During the school year, vision, hearing, dental and general health screening tests will be done for kindergarten
and/or 1st, 2nd, 5th and 8th grade students. Students attending Special Education classes
will also be given the screening tests. Students in grades 3, 4, 6, and 7 will receive the above services
on teacher or parent referral only.
Parents will be notified if their child is identified as needing referral for diagnostic and health care
services. Notice will not be sent to parents if their child successfully completes all of the health
screening tests.
If you wish to have your child exempt from this testing, you must notify your child’s school in writing.
This notice is in compliance with §10921 of the State of California Education Code.
4. School Nurse Consultations
Please call your child’s school for an appointment to talk with the Nurse if your child has had any
health concerns the Nurse should be aware of, or you would like a hearing or vision exam done on
your child.
